Translating Evidence into Practice

Over the past eight weeks, through your weekly assignments, you followed the steps to develop a practice improvement project. Beginning with the burning question, you searched the literature to determine the state of the science on the topic. After analyzing the evidence, the best evidence became the intervention you would implement and evaluate as you translate research into practice. This assignment represents a refinement of your work done, incorporating feedback from your instructor.

Compile your work from the previous assignments into a 10-page paper (final project), including the following elements:

  • The background question
  • The PICOT question
  • The model you will use to guide your project and change practice, including a brief rationale for that choice
  • The Search Tracker that includes the PICOT question and delineates the steps of your search process (included in an appendix)
  • Evaluation and Synthesis Tables based on the pared-down search, with a narrative that explains the results and includes:
    • What the evidence tells you about best practices for this topic
    • How the body of evidence will help you design your practice improvement project
    • Outcomes that you will measure to determine whether change has occurred, their sources (electronic health record [EHR], deaths, etc.), and how the data will be evaluated (this may be in the form of a grid in the appendix)
  • The change model you will use to motivate stakeholders with a brief rationale.

The 10-page paper (excluding the cover page, references, the Evaluation Table, the Synthesis Table, and the outcomes grid) should be double spaced and APA formatted.
