Scenario

A nurse is caring for an elderly patient who has just been discharged from the hospital. The patient is hesitant to go to follow-up appointments because of the difficulty in transportation. The nurse is worried that the patient will have difficulty managing their health condition and want to ensure the safety of the patient after discharge.

Students will be asked to analyze the scenario in the context of the QSEN framework and identify how the Quad Council Competencies can be used to address the patient’s safety problems. They must also reflect on their own experiences and observations related to the quality and safety of healthcare.
