COMMUNITY NEEDS ASSESSMENT PLAN—PRIORITIES, TIMELINES, AND DECISION-MAKING PROTOCOLS

    Week 4

    Social Problem: Homelessness

Homelessness is a complex social issue affecting individuals and families, often rooted in factors such as lack of affordable housing, unemployment, mental health issues, and substance abuse. Addressing homelessness requires a multifaceted approach that provides immediate relief while also working toward long-term solutions.

    Top Five Prioritized Strategies

    1. Developing a Comprehensive Needs Assessment

    · Reasoning: Understanding the specific needs of the homeless population in the community is crucial. A thorough needs assessment will identify the demographics, underlying causes, and necessary resources.

    · Time and Resources: This can be done within a few months with minimal financial resources by collaborating with local organizations and volunteers.

    2. Establishing Partnerships with Local Organizations

    · Reasoning: Collaborating with existing shelters, food banks, and health services can maximize resources and provide immediate support to the homeless community.

    · Time and Resources: Building partnerships requires time for meetings and negotiations but is cost-effective since many organizations are eager to collaborate.

    3. Creating Emergency Shelter Options

    · Reasoning: Providing immediate shelter is essential to prevent further deterioration of the situation. This can reduce exposure to harsh weather and increase safety.

    · Time and Resources: Setting up temporary shelters can be implemented quickly using community spaces (e.g., churches or community centers) and requires relatively low funding compared to permanent housing solutions.

    4. Implementing Job Training and Employment Programs

    · Reasoning: Employment is a key factor in preventing homelessness. Providing job training can help individuals gain skills and secure stable jobs.

    · Time and Resources: This requires more time and resources to develop programs and partnerships with local businesses, but it has the potential for significant long-term impact.

    5. Advocating for Affordable Housing Initiatives

    · Reasoning: Long-term solutions require addressing the root cause of homelessness, which is often a lack of affordable housing. Advocating for policies that support affordable housing development is crucial.

    · Time and Resources: This is a longer-term strategy that involves advocacy work and policy change, which can take years. However, it leverages community efforts and can attract funding from various sources.

    Conclusion

The prioritization of these strategies is based on a balance of immediate impact and long-term sustainability. By starting with a needs assessment and building partnerships, we can establish a solid foundation for more extensive programs like job training and housing advocacy. Each strategy builds on the others, creating a comprehensive approach to addressing homelessness effectively

  • Responding to Abnormal Physical Assessment Findings With Culturally Competent Care  One goal of this Competency is to acknowledge your experience and to help you examine

Responding to Abnormal Physical Assessment Findings With Culturally Competent Care

One goal of this Competency is to acknowledge your experience and to help you examine, interpret, and learn from it in ways that advance your nursing skills. This Competency also puts priority on addressing aspects of nursing that are inevitable yet can be inherently challenging.

With those goals in mind, your Competency Assessment requires an example of an abnormal finding in the physical assessment of a patient. You are encouraged to draw from your own experience. However, you may also work with a focus case obtained from another professional nurse. Consider details, such as the following, that you will need to provide:

    • What was the purpose of the exam?
    • How was the abnormality found?
    • How was it determined to be abnormal?
    • Was the abnormal finding usual/unusual, otherwise characterized, based the patient’s age/place in the lifespan?
    • How was the finding shared with the patient?
    • What were next steps for diagnosis and/or treatment?

Explaining the role of cultural competence in nursing care is an equally important component of this Assessment. You will analyze the impact of cultural competence in conducting a physical assessment and responding to an abnormal finding. You will explain how a nurse’s demonstration of cultural competence would improve the patient’s experience and potential outcomes. For these requirements, you may incorporate details about your focus patient that you know. Or, you may approach this part of the Assessment by presenting best practices.

Your Performance Task is to draw this information on cultural competence and physical assessment together in a PowerPoint presentation to inform nurses in a practice setting on providing culturally competent care and the particular impact with physical assessments that have abnormal findings. (Note: You are not required to include audio narration.) Be sure to adhere to rules of confidentiality and omit identifying details about the patient, healthcare setting, or other healthcare providers.

Note: Be sure to delete the formatting notes and tips in the PowerPoint template. Your presentation should feature only your original content and wording.

    To complete the Competency Assessment:

Using the PowerPoint template document, create a PowerPoint presentation of 11–14 slides, plus a title slide and reference slide(s), to include the following:

Part 1: Providing Culturally Competent Care

    • Explain what “cultural competence” means in professional nursing practice.
    • Describe examples of how culturally competent care benefits patients and patient outcomes.
    • Explain practical approaches to demonstrating cultural competence, including cultural humility, in nursing care.
    • Explain how knowledge of a patient’s culture, including norms and beliefs, is important in a physical assessment, and why.
    • Identify a patient who is the focus of your presentation with appropriate details: the patient’s age, cultural background, health background, and purpose of the physical assessment.

Part 2: Responding to an Abnormal Finding in a Physical Assessment

    • Explain what you found in the physical assessment of the patient, how you made the identification, and why the finding was abnormal.
    • Based on the patient’s age and place in the lifespan, explain how you characterize this finding, and why.
    • Explain how knowledge of the patient’s cultural norms and beliefs supports presenting the abnormal finding to the patient.
    • Explain your decision making on next steps for the patient.
    • If you were making a referral for this patient, explain what you would consider, and why.
    • Explain the impact and value of this experience on your nursing practice, with specific examples.
  • Advocating to Educate on Community Resources to Support Social Determinants of Health What are significant social needs in your community and how can you help? Through your professional role as a nurse

    INSTRUCTIONS

Advocating to Educate on Community Resources to Support Social Determinants of Health

What are significant social needs in your community and how can you help? Through your professional role as a nurse, as well as personal interactions as a community member, you likely have insight into issues that impact certain populations. Chances are you also see from your daily interaction with patients how lack of information or information that is not easily accessible or understandable prevents patients from making changes or getting resources and support that would improve their lives.

Using your knowledge of the social determinants of health and the needs of particular groups, you can be a significant advocate for a healthier community. Think of yourself as a bridge that connects those who need resources with those who can provide them. Every organization can use another voice to spread the word about what they do and offer. Vulnerable populations will benefit from an advocate who is aware of potential barriers, such as language or literacy levels, in recommending community resources.

For this Competency Assessment, you will identify a particular population in your community. You will research this population to understand their specific characteristics and needs and match them to organizations and services in your community. In addition, you will identify missing services to be developed and make the case for strengthening existing services. You will organize your research using a PowerPoint presentation. (Note: You are not required to add audio narration.) Think of your presentation as a place to start in determining how best to reach and inform your focus population and in filling your role as a social change agent with the information you gather.

    To prepare:

    • Access the PowerPoint template document for your presentation. Reflect on how to create a presentation that could reach and inform stakeholders in your healthcare facility and your community.
    • Research to identify specific needs in your community based on the social determinants of health (SDoH): economic stability, education quality and access, healthcare access and quality, neighborhood and built environment, social and community context.
    • Identify a population (e.g., children, families, the elderly, those with a particular health condition, residents of a particular neighborhood) with specific SDoH needs that you will advocate for as a social change agent in your community. Research to identify key characteristics of this population that are important in identifying resources for them, such as age, language, cultural considerations, technology access, literacy levels, and so on.
    • Research resources in your community (e.g., organizations and services)  you would recommend to the target population to address their specific SDoH needs and improve health and well-being. Aim for resources that will address each SDoH as it impacts your focus population or the SDoH(s) of greatest need. Identify all important information that the population will need to access and benefit from each resource to include the following: location, hours, languages spoken/multilingual materials available, special services, free services/materials, online links. Make a point to learn as much as you can about the resource(s) you highlight, including to determine ways that services could be improved to better meet your population’s needs.
    • Based on your research, identify services that are missing in the community and needed by your population.
    • Consider strategies for reaching the focus population that align with characteristics of the population (e.g., technology access and forms, such as text, email, or print/poster options).
    • Based on your advocacy plan, formulate goals for improving health outcomes for this population.
    • Note: The PowerPoint template includes formatting notes and tips. Be sure to delete this information from your slides. Your presentation should feature only your original content and wording.

    To complete the Competency Assessment:

Using the PowerPoint template document provided, create a PowerPoint presentation of 10–12 slides, plus title and reference slides, to include the following:

Part 1: Planning for Social Change Advocacy

    • Summarize your concerns as a professional nurse about health quality and equity in your community.
    • Explain your goals as a professional nurse to be a social change agent in your community. Be specific.

Part 2: Advocating for a Population Based on Social Determinants of Health

    • Identify the population in your community for which you will advocate. Describe key characteristics of this population (e.g., age, language, culture). Be specific with examples.
    • Identify the population’s needs based on the social determinants of health (SDoH). Include examples to explain these needs.
    • Identify three (3) resources (i.e., organizations and services) in your community that you would recommend to this population to address their specific needs, as well as improve health and well-being as defined by the SDoH. Explain your rationale for selecting each resource.
    • Describe essential information the population would need to access and benefit from each resource (e.g., location, hours, languages spoken/multi-lingual materials available, special services, free services/materials, online links).
    • Identify at least one example of a missing resource that is needed by the population and why.
    • Explain recommendations for improving existing services to better meet particular needs.

    Part 3: Reaching the Population

    • Describe strategies for reaching the population with the community resource information you identified and that align with the characteristics and preferences of this population. Be specific with examples.
    • Based on your advocacy plan, identify goals for improving health outcomes for this population.
